ban with reasons and add a reason in ESC if wanted and ban time menu (eXtensible Admin)

Well the title says all.

Add reasons to the ban and if wanted add a custom reason just by using ESC.

And a simple ban time menu with options to ban by IP or STEAMID and time ban like:

5 m ban
30 m ban
1hr bam
24 ban
perma ban
